Please follow these instructions carefully to ensure your form is processed without delay.
How to Submit:
1. Email your form as a PDF attachment only: We do not accept photos or screenshots taken with a phone.
2. Send your email to: forms@totalcareped.com
3. In the email, include the following required information:
• Child’s full name
• Date of birth (DOB)
• Home address
Forms missing any of the above information or sent in the wrong format may be delayed or returned.
Completed forms will be returned within 7 business days.
